1. Technical Documents

  • Filled-out Unified Application Form for Certificate of Occupancy and Fire Safety Inspection Certificate (FSIC); (3 original copies)
  • Fire Safety Evaluation Clearance (FSEC) (1 photocopy)
  • Approved Building Plans (1 set of original/ photocopy)Note: A3 size or Blueprint Only
  • Approved Permits (1 photocopy)

    • A. Building Permit
    B. Electrical Permit
    C. Plumbing/Sanitary Permit
    D. Electronics Permit (if applicable)
    E. Mechanical Permit (if applicable)
    F. Fencing Permit (if applicable)
    G. Sign Permit (if applicable)

  • Filled-out Certificate of Completion Form for Certificate of Occupancy; signed, sealed, and duly notarized (3 original copies)
  • One (1) photocopy of valid PRC ID & current PTR with seal and 3 specimen signatures of the professional
  • Colored photos of the completed structure showing Front, Sides, and Rear Areas (1 original copy)
  • Construction Logbook, signed and sealed by the Architect or Civil Engineer for full-time inspection and conformed by the owner (1 original copy)

    • 2. Legal Documents

  • If a Representative is transacting and/or signing in behalf of the owner:
    • Notarized Special Power of Attorney (SPA) (1 original copy) with Valid ID of client and representative. (1 photocopy)
  • If a representative is sent to receive the certificate in behalf of the owner:
    • Authorization of the owner (1 original copy) with Valid ID of client and representative. (1 photocopy)
  • If the signatory in the Certificate of Completion is not the same with what is in the building/ancillary permits:
    • Notarized Waiver (1 original copy)

    Reminder:

      -ALL NECESSARY DOCUMENTS MUST BE COMPLETED, SIGNED, AND SEALED BY PROFESSIONALS.

      -THE GENERATED APPLICATION FORMS SHOULD BE PRINTED BACK TO BACK.

  • One (1) Pc Long size folder with fastener:

    • BLUE FOLDER – Complex Applications
    Complex application comprising of:
    1) Interior Renovations covered by an appropriate Building Permit
    2) Conversion of existing building occupancy classification
    3) Multiple unit residential houses (such as row houses, townhouses, and the like)
    4) Single detached residential house, not more than 7 storeys
    5) Commercial buildings not more than 9 storeys
    6) Warehouses not more than 9 storeys
    7) Market buildings not more than 9 storeys
    8) Demolition (any type of occupancy/ use with area of more than three hundred (300) square meters)
    9) Billboard structures; 10) All other types of Occupancy not more than 9 storeys and communication towers (based on JMC 1, s. 2020)

    Green Folder – Highly Technical Applications

    Highly Technical applications comprising of:
    1) Commercial buildings exceeding 9 storeys
    2) Market buildings exceeding 9 storeys
    3) All applications belonging to Group D of NBCP
    4) All applications belonging to Group H and I of NBCP
    5) All other types of occupancy of more than 9 storeys
    6) Special structures such as but not limited to Aerodome facilities, before construction work or operation can commence in compliance with the National Building Code of the Philippines (PD 1096), its IRR and Referral Codes, other pertinent laws, and directives.
